Alfonzo B. “Alfy” Qualls, Sr., 36, of Austin, Indiana, passed away on Saturday, April 30, 2016 in an automobile accident near Lexington, Indiana. He was born on October 18, 1979 in Chicago, Illinois, the son of Alfonzo Casarrubias and Judy (Qualls) Salgado. Alfy was an employee of Burton Lumber Company in Blocher, Indiana, also formerly worked at Austin Tri-Hawk and enjoyed playing with his kids. He was killed in the same accident as his daughter, Mariah Qualls and stepson, Caden Pedigo. Survivors include his wife, Christine N. (Dean) Qualls; a son, Alfonzo Qualls, Jr. of Lexington, Indiana; two daughters, Jasmine Qualls of Lexington, Indiana and Mikayla Qualls of Austin, Indiana; Father and Stepmother, Alfonzo and Priscilla Casarrubias of Evergreen Park, Illinois; his mother, Judy Salgado of Tennessee; two sisters, Estella and Heidi Casarrubias both Georgia; a brother, Alfonzo Casarrubias, Jr. of Chicago, Illinois, several other brothers, sisters and cousins.

Cremation was entrusted to Collins Funeral Home in Scottsburg, Indiana and all services will be private.

To those who knew Bradley know what a good dad he was and a great friend that would give his shirt off his back. For those who didn't know Bradley I'm deeply sorry because you definitely missed out on a great guy that had alot of love for alot of people especially his friends and family. Bradley was the kind of guy all you had to do was ask and he was there, " I just wish I could ask one more time". This guy will truly be missed by many. Rest in piece my brother.
