Hazel Blankenship

Hazel Blankenship, 64, of Lexington, Indiana, passed away on Saturday, November 21, 2015 at Jewish Hospital in Louisville, Kentucky.  She was born in Deputy, Indiana, the daughter of the late Robert and Otie (Turner) Johnson.  Hazel was retired from Hardy Lake in Austin, Indiana and also formerly worked at Scott Memorial Hospital and Austin Elementary School and graduated from Scottsburg High School in 1969.  She was a certified clown for a local clown ministry and was known as Blossom the Clown, enjoyed working in her flower garden and loved animals. Hazel was preceded in death by a grandchild, Tristan Blankenship and a brother, Kent Johnson. Survivors include her husband, John L. Blankenship; two sons, James Shawn (Amy) Blankenship of Seymour, Indiana and Christopher Lee Blankenship of Austin, Indiana; two sisters, Irene Howard of Austin, Indiana and Louise Rice of Columbus, Indiana; two brothers, R.J. Johnson of Scottsburg, Indiana and George Johnson of Austin, Indiana; a granddaughter, Mikaela Nelson (Carl) and a great grandson, Isacc Morgan. Online condolences: www.collinsfuneralhome.net

Funeral Service:
6:00 pm Monday, November 23, 2015 at Collins Funeral Home with Pastor Judy Howard-Stater officiating.
Visitation:
3 to 6 pm Monday at Collins Funeral Home. Cremation is requested following the services. Memorial Contributions: To
