Mariah Rae Qualls, 8, of Lexington, Indiana, passed away on Saturday, April 30, 2016 in an automobile accident near Lexington, Indiana. She was born on March 11, 2008 in Louisville, Kentucky, the daughter of the late Alfonzo B. Qualls Sr. and Bernice Takacs. Mariah was a first grade student at Lexington Elementary School. She was killed in the same accident with her father, Alfonso Qualls, Sr. and stepbrother, Caden Anthony Pedigo. Survivors include her mother, Bernice Takacs; a brother, Alfonzo Qualls, Jr. of Lexington, Indiana; two sisters, Jasmine and Renesmer Qualls both of Lexington, Indiana; a half sister, Mikayla Qualls of Austin, Indiana; Grandparents, Kenneth and Yolanda Jennings of Lexington, Indiana, Alfonzo Casarrubias of Evergreen Park, Illinois and Judy Salgado of Tennessee; a Great Grandmother, Dorothy Jennings of Scottsburg, Indiana and numerous aunts, uncles and cousins. Mmorial Contributions: The Family c/o Collins Funeral Home in Scottsburg, Indiana.

Funeral Service:
1:00 pm Friday, May 6, 2016 at Collins Funeral Home in Scottsburg, Indiana with cremation requested following the services.
Visitation:
11 am to 1 pm Friday at Collins Funeral Home.
